Abstract

It has long been acknowledged that current factors working against health institutions in Nigeria have an impact on medical record documentation. The State Specialist Hospital Asubiaro, Osogbo has a track record of competent Health Information officers who use various documentation methods. The methods in use are evaluated while also identifying the faults or variables that impede efficient documentation. . A random sample technique was used, which is a sub-group of people chosen by chance in such a way that everyone has a chance of being chosen. 120 questionnaires were distributed and collected by hand. According to the data gathered, the most significant reason impeding successful MRD in State Specialist Hospital Asubiaro, Osogbo is a lack of resources and financing, as well as complaints about non-legibility of physicians’ handwriting and a shortage of qualified employees. Based on the findings of this study, it was determined that insufficient funds and resources are the primary impediment to effective MRD at the State Specialist Hospital in Asubiaro, Osogbo. To improve efficient MRD in State Specialist Hospital Asubiaro, Osogbo, the government should provide additional facilities and electronic medical record equipment, as well as make an effort to hire more Health Information experts.

There are several health-care delivery systems in use around the world. Nations, on the other hand, build and develop health-care systems that are tailored to their own requirements and resources. Primary healthcare is a health system that consists of the planning and distribution of health care needs among market participants, while public health care is an effort by governments, trade unions, charities, religious organizations, or other coordinated bodies to deliver planned health care services targeted to the populations they serve.

The design and distribution of health care is more evolutionary than revolutionary.

Health systems, according to the World Health Organization, are made up of all organizations, individuals, and actions aimed at promoting, restoring, or preserving health. This encompasses actions that improve health and initiatives that alter health determinants. A health system includes publicly owned facilities that provide personal health services, health insurance companies, a mother caring for a sick kid at home; vector-control initiatives, private health-care providers’ efforts; behavior modification programs, and workplace health and safety legislation.

It also considers inter-sectoral activity by health professionals, such as boosting the ministry of education to promote female education, which, according to the World Health Organization, facilitates the growth of good health (2000).

The documentation of the health-care delivery system is crucial for providing the necessary information on the evaluation and treatment of health-care services, as well as transmitting such information to third-party payers. Many additional insurance plans use documentation criteria as a guideline. Documentation also contains essential clinical data. It is critical that the records, notes, and reports are clear, legible, and convey all of the necessary information for clinical management and reimbursement. Inadequate skilled people, inefficient technology, a lack of financial resources to implement new technology that promotes good documentation, and a lack of information technology ability are among the challenges that documentation faces.
